Early Intervention Program for Infants and Toddlers - Waianae

Background
The State of Hawai‘i, through its Department of Health (HDOH), Family Health Services Division (FHSD), Children with Special Health Needs Branch (CSHNB), Early Intervention Section (EIS), is seeking proposals for contracted Purchase of Service (POS) Early Intervention (EI) Programs.

These programs are aimed at providing family-centered, community-based EI services for infants and toddlers from birth to age three who have developmental delays or diagnosed conditions likely to result in such delays. The goal is to enhance the development of these children and support their families in meeting their needs, in compliance with the Individuals with Disabilities Education Act (IDEA) and related state laws.

Work Details
The contracted EI services include:
- Care coordination
- Family training, counseling, and home visits
- Occupational therapy
- Physical therapy
- Social work
- Special instruction
- Speech language pathology

Services must align with each child's Individualized Family Support Plan (IFSP). The contract will cover various geographic service areas within the State of Hawai‘i, and applicants must submit separate proposals for each area they wish to serve. The contract term is from July 1, 2025, through June 30, 2026, with options for extensions up to a maximum of five years.

Period of Performance
The initial contract term is from July 1, 2025, to June 30, 2026. There are provisions for four one-year extensions, allowing a maximum contract duration of five years until June 30, 2030.

Place of Performance
The services will be performed in various geographic service areas throughout the State of Hawai‘i.
