Description

The Middle Georgia Regional Commission (MGRC) is soliciting proposals from potential Offerors interested in providing Evidence Based Wellness services for the following counties: Baldwin, Crawford, Houston, Jones, Macon-Bibb, Monroe, Peach, Pulaski, Putnam, Twiggs, and Wilkinson from July 1, 2025 June 30, 2028.
Background
The Middle Georgia Regional Commission (MGRC) is seeking proposals for Evidence-Based Wellness services aimed at improving the health and well-being of older adults in the region.

The MGRC's mission is to assist in developing programs that support older individuals and their caregivers, ensuring they lead healthy, independent lives. This RFP is part of their efforts to enhance community planning and service delivery systems for aging services across multiple counties.

Work Details
The selected Offeror will provide Evidence-Based Wellness services across the following counties: Baldwin, Crawford, Houston, Jones, Macon-Bibb, Monroe, Peach, Pulaski, Putnam, Twiggs, and Wilkinson.

Services may include health promotion activities tailored to older adults and their caregivers. The MGRC may award contracts to providers for individual counties or combinations thereof based on proposals received. Specific services required are detailed in the appendices of the RFP.

Period of Performance
The contract will commence on July 1, 2025, and conclude on June 30, 2028.

Place of Performance
Services will be performed in the counties of Baldwin, Crawford, Houston, Jones, Macon-Bibb, Monroe, Peach, Pulaski, Putnam, Twiggs, and Wilkinson.
