Request for Quotes- Middle School Flooring

Background
The Habersham County School District is seeking quotes for the removal and replacement of existing carpet with luxury vinyl plank flooring at three middle schools: Hilliard Wilbanks Middle School, North Habersham Middle School, and South Habersham Middle School. The goal of this contract is to enhance the flooring in these educational facilities, ensuring a modern and durable environment for students and staff.

Work Details
The project involves the following specific tasks:

1. Removal of existing carpet at the following locations:
- Hilliard Wilbanks Middle School, 3115 Demorest Mt Airy Hwy, Demorest, GA 30535
- Flooring to be replaced with Shaw Terrain #568 Root and cove base Johnsonite DC-32 Pebble 4”.
- North Habersham Middle School, 1500 Wall Bridge Rd, Clarkesville, GA 30523
- Flooring to be replaced with Tarkett Provence and cove base Roppe Lunar Dusk #114, 4”.
- South Habersham Middle School, 237 Old Athens Hwy, Cornelia, GA 30531
- Flooring to be replaced with Tarkett Coriander and cove base Roppe Taupe #124, 4”.

The contractor must furnish all materials, labor, equipment, and other services necessary for the performance of this contract. Location plans are provided separately with marked areas for removal and replacement.

Period of Performance
The work is anticipated to begin as early as May 29, 2025, with a completion date set for July 15, 2025.

Place of Performance
The contract will be performed at three locations within Habersham County: Hilliard Wilbanks Middle School in Demorest, North Habersham Middle School in Clarkesville, and South Habersham Middle School in Cornelia.

Bidder Requirements
Bidders must provide a current certificate of insurance with specified coverage amounts. They must also submit an E-Verify Proposer Affidavit confirming compliance with federal work authorization programs. Additionally, bidders need to comply with the Immigration Reform and Control Act of 1986 and provide valid licenses to operate in Georgia. References (one personal and two business) must be included along with a current W-9 form.
