The Department of Community Health (DCH) of the State of Georgia is seeking qualified Suppliers to provide services for the Program of All-Inclusive Care for the Elderly (PACE). This innovative healthcare delivery model aims to assist seniors in maintaining their health and independence while living in their communities.

The PACE program offers comprehensive medical and social services tailored to meet the specific needs of eligible senior participants, ensuring availability 24/7 throughout the year. The goal is to establish PACE organizations across multiple service regions in Georgia, facilitating a three-way agreement with the Centers for Medicare and Medicaid Services (CMS) for operational readiness within 19-24 months post-award.

Work Details
Suppliers are required to provide PACE organization services that include:
- Initiating the PACE application process with CMS for site approval covering awarded service regions.
- Establishing a fully operational PACE center at their own expense until CMS approval is obtained.
- Ensuring compliance with Georgia Care Management Organization (CMO) time and distance requirements across selected zip codes.
- Responding to proposals for Core Service Regions, which include specific zip codes in areas such as Bibb County, Chatham County, Dekalb County, and Fulton County. Each service region will be awarded to one PACE organization, with potential exceptions allowing multiple awards in certain regions like East Point/College Park where two centers must be established.

Period of Performance
The contract period is expected to commence upon award, with operational readiness targeted within 19-24 months after contract award.

Place of Performance
Services will be performed in designated Core Service Regions across Georgia, including but not limited to East Point/College Park, Decatur, Macon, Columbus, Alpharetta, Savannah, Athens, Augusta, Gainesville, Lawrenceville/Gwinnett, Marietta, Metro East, and Metro West.

Bidder Requirements
Suppliers must demonstrate fiscal soundness and resources necessary to operate a viable PACE program. They must also have a historical commitment to serving the elderly population and comply with all CMS regulations regarding PACE operations.
