Description

Pursuant to the State Purchasing Act (Official Code of Georgia Annotated 50-5-50 et seq.), this electronic Request for Quotes (eRFQ) is being issued to establish a contract with one or more qualified suppliers who will provide Newborn Screening (NBS) Forms to the Georgia Department of Public Health (hereinafter, the State Entity) as further described in this eRFQ. The Contractor shall supply newborn screening specimen collection forms printed by printers that operate through a Federal Food and Drug Administration (FDA) approved facility meeting the International Organization for Standardization (ISO) 9001-2008 certification, as specified in the agreement and the eRFQ.
Background
The Georgia Department of Public Health (DPH) is issuing this electronic Request for Quotes (eRFQ) to establish a contract with qualified suppliers for the provision of Newborn Screening (NBS) Forms. The purpose of this procurement is to secure high-quality, cost-efficient services and exemplary customer service for the Georgia Public Health Laboratory (GPHL), which performs mandated newborn screenings as per Georgia law. The GPHL requires NBS forms printed by FDA-approved facilities that meet ISO 9001-2008 certification standards.

Work Details
The contractor will supply newborn screening specimen collection forms that are printed by printers operating through an FDA-approved facility. These forms must be made from six-part no carbon required (NCR) filter paper and certified by the Centers for Disease Control and Prevention (CDC). The contractor is required to maintain an emergency supply of at least 10,000 forms, ready for shipment within 24 hours upon request from the Department. The forms must have a shelf life of no less than 24 months.

Period of Performance
The initial term of the contract is one calendar year from the execution date, with four optional one-year renewals at the discretion of the State Entity.

Place of Performance
The contract will be performed at the Georgia Public Health Laboratory located at 1749 Clairmont Road, Decatur, GA 30033.

Bidder Requirements
Suppliers must be registered in Team Georgia Marketplaceā„¢ and comply with all insurance requirements, including Workers Compensation Insurance and Commercial General Liability Insurance. Additionally, suppliers must provide proof of FDA approval and ISO certification for their printing facilities.
