Background
The Florida Agency for Health Care Administration is issuing this Request for Applications (RFA) to establish grant agreements with eligible subrecipients under Florida’s Rural Health Transformation Program (RHTP). This federally funded initiative aims to strengthen rural health care facilities' capacity to deliver specialty, acute, and behavioral health care through shared technology platforms and expanded telehealth services.
The RHTP is organized around five strategic goals: Make Rural America Healthy Again, Sustainable Access, Workforce Development, Innovative Care, and Tech Innovation. The Specialty and Acute Care Bundle (Bundle Two) consists of two required core service components and four elective telehealth initiatives designed to enhance rural healthcare delivery.
Work Details
The contract includes the following core service components:
1) Florida HIE/ENS Onboarding (Initiative 13) - $2,500,000;
2) Diagnostics Technology Support (Initiative 14) - $9,130,531.
Additionally, applicants must select at least one of the following elective telehealth initiatives:
3) Behavioral Health Telehealth & Telehub Psychiatry (Initiative 4) - $18,000,000;
4) Tele-Imaging / Virtual Specialty Clinics (Initiative 5) - $16,000,000;
5) Tele-Intensive Care Unit / eICU (Initiative 6) - $14,000,000;
6) Hub-and-Spoke Telestroke (Initiative 7) - $10,000,000.
Each application must include both core components and at least one elective initiative.
Period of Performance
The period of performance for the contract is from December 29, 2025 to October 30, 2030. Year 1 will begin on August 1, 2026 or upon date of execution and will end on July 30, 2027.
Place of Performance
The services will be performed across various rural locations in Florida as specified in the application process. Specific counties eligible under the RHTP rural eligibility pathways must be documented in applications.
Bidder Requirements
Applicants must demonstrate organizational capacity to deliver both core service components and any additional telehealth initiatives proposed. They are required to establish formal partnerships with organizations that can deliver components not directly operated by the lead entity.
Each application must include executed MOUs or Letters of Intent for all named partner organizations and subrecipients at the time of application.
