Background
The Florida Agency for Health Care Administration is issuing this Request for Applications (RFA) to establish grant agreements under the Rural Health Transformation Program (RHTP), a five-year federally funded initiative aimed at expanding access to primary and preventive care in Florida's rural communities.

Work Details
The Rural & Satellite Clinics Initiative will provide funding for startup capital, health information technology (HIT) integration, workforce training, limited minor renovations, and regional collaboration support necessary to establish sustainable rural clinic operations.
Key program components include:
- Startup funding for new rural clinics or satellite extensions of existing providers.
- Integration of health information technology including EHR/EMR systems and telehealth infrastructure.
- Recruitment and training for clinical staff.
- Care coordination functions and patient navigation.
- Quality assurance systems and performance monitoring.
- Minor renovations to prepare clinic spaces for patient care, subject to CMS prior approval.
Period of Performance
The period of performance for this initiative is from December 29, 2025, to October 30, 2030, with Year 1 specifically running from August 1, 2026 (or date of execution) to July 30, 2027.
Place of Performance
The contract will be performed in Florida's eligible rural areas as defined by the RHTP framework.
Bidder Requirements
Eligible applicants include Rural Health Clinics (RHCs), Federally Qualified Health Centers (FQHCs), Critical Access Hospitals (CAHs), County Health Departments (CHDs), behavioral health centers adding integrated primary care services, physician groups, dental practices providing primary oral health services in rural areas, nurse practitioner-led practices, educational institutions establishing clinical training sites that provide direct patient care, and Managed Care Organizations (MCOs).
Applicants must demonstrate a five-year operational history delivering healthcare services and must meet specific minimum qualifications including active registration with SAM.gov.
