RHTP Preventive Care and Care-at-Home (Bundle One)

Background
The Florida Agency for Health Care Administration is issuing a Request for Applications (RFA) to establish grant agreements under the Rural Health Transformation Program (RHTP). This federally funded initiative aims to enhance rural health care through five strategic goals: Make Rural America Healthy Again, Sustainable Access, Workforce Development, Innovative Care, and Tech Innovation.

The Preventive Care and Care-at-Home Bundle (Bundle One) encompasses multiple initiatives that must be delivered as an integrated program, focusing on creating a coordinated continuum of rural preventive care and care-at-home services.

Work Details
The Preventive Care and Care-at-Home Bundle includes five required initiatives:
1) Mobile Health Units to identify patients in community settings;
2) Community Paramedicine for clinical follow-up in rural homes;
3) Remote Patient Telemonitoring for chronic disease oversight;
4) Retail Pharmacy Clinics as accessible non-emergency care sites;
5) HIE/ENS Onboarding for shared data infrastructure.

Each initiative must be fully integrated into the application, with specific funding allocations: Mobile Health ($20M), Community Paramedicine ($18M), Remote Patient Telemonitoring ($14M), On-Site Pharmacy/Retail Clinic Services ($6M), and HIE/ENS Onboarding ($2.5M). The total funding available for Year 1 is $60.5 million statewide.

Period of Performance
The contract period of performance spans from December 29, 2025, to October 30, 2030, with Year 1 specifically from August 1, 2026 (or date of execution) to July 30, 2027.

Place of Performance
The services will be performed across various rural counties in Florida as defined by the Super Regions established under the RHTP.

Bidder Requirements
Eligible applicants include Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), Critical Access Hospitals (CAHs), County Health Departments (CHDs), licensed primary care practices serving rural areas, Community-Based Organizations with clinical oversight arrangements, Area Agencies on Aging, Federally recognized tribal organizations, and regional healthcare collaboratives.

Applicants must demonstrate organizational capacity to deliver all five required bundle components and establish formal partnerships where necessary.
