Background
The Florida Agency for Health Care Administration is issuing a Request for Applications (RFA) to establish grant agreements under the Rural Health Transformation Program (RHTP). Eligible applicants include Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), Critical Access Hospitals (CAHs), County Health Departments (CHDs), licensed primary care practices serving rural areas, Community-Based Organizations with clinical oversight arrangements, Area Agencies on Aging, Federally recognized tribal organizations, and regional healthcare collaboratives.
Applicants must demonstrate organizational capacity to deliver all five required bundle components and establish formal partnerships where necessary.
