201 Building Suites 200 & 210

Background
Halifax Hospital Medical Center, doing business as Halifax Health, is a special taxing district hospital that operates multiple inpatient facilities and outpatient centers in Florida. The organization is seeking qualified architectural and engineering firms to provide complete design services for the minor renovation and interior enhancement of Suites 200 and 205/210 within the Halifax Health 201 Building, located at 201 N. Clyde Morris Boulevard, Daytona Beach, Florida. The goal of this contract is to improve the functionality and aesthetics of these spaces to better serve patients and staff.

Work Details
The scope of work includes:
- Selective interior demolition;
- Renovation and reconfiguration of clinical, administrative, and support spaces;
- Upgrades to flooring, walls, ceilings, lighting, finishes, casework, millwork, doors, glazing, and window treatments;
- Modifications to nurse stations, workstations, exam rooms, offices, labs, conference areas, and restrooms;
- Relocation or addition of power, data, and plumbing to support new layouts and equipment;
- Restroom renovations and ADA compliance modifications;
- Installation of new lighting systems with dimming controls;
- Replacement of existing ceiling systems;
- Improvements to storage and lab areas;
- Selective repair or reinforcement of existing partitions;
- Removal and replacement of select building components such as curtain tracks and doors;
- Preparation and refinishing of existing surfaces where required. The selected firm will be responsible for developing complete construction documents suitable for permitting and construction.

Period of Performance
The estimated period of performance is three years from the date the Notice of Intent to Award is posted with an option for one additional year.

Place of Performance
The contract will be performed at the Halifax Health 201 Building located at 201 N. Clyde Morris Boulevard, Daytona Beach, Florida.

Bidder Requirements
Proposers must be registered to conduct business in the State of Florida. They must submit a proposal with all required documentation including proof of applicable licenses. Proposers are also required to provide:
- A Sworn Statement on Public Entities Crimes (Attachment A)
- Scrutinized Company Certification (Attachment B)
- Foreign Countries of Concern attestation (Attachment C)
- Human Trafficking Attestation (Attachment E)
- Proposer References (Attachment F)
- Copies of current business licenses
- Verification of business registration with Florida Department of State (Sunbiz)
- A copy of current Certificate of Insurance for reference purposes only
- A completed IRS Form W9.
