Background
The Department of Developmental Services (DDS) is seeking proposals from private providers to develop, operate, supervise, and manage the administrative duties associated with the Community Clinical Response (CCR) program.
The goal of this contract is to provide comprehensive support within family homes to enhance the well-being of individuals receiving services.
Work Details
The successful proposer will be required to:
1. Provide supports within the family home.
2. Complete a functional behavioral assessment (FBA).
3. Develop a positive behavior program tailored to individual needs.
4. Train family members on implementing the behavior plan effectively.
5. Model program implementation for families and provide ongoing consultation to other in-home support providers.
6. Develop a transition plan for discharge from services as needed.
7. Provide nursing consultation regarding medication and medical needs if required.
Period of Performance
The contract will be performed from January 13, 2026, to April 07, 2026.
Place of Performance
The services will be performed in family homes located in Connecticut.
