Prescription Eyeglasses for Inmates

Background
The Colorado Department of Corrections (CDOC), Division of Clinical Services, seeks to enter into a contract or purchase order with a vendor to provide high-quality prescription eyeglasses for inmates located in correctional facilities throughout Colorado. The total dollar amounts available for any resulting contract or purchase order from this RFP is expected to be approximately $100,000 annually. No dollar amounts or quantities are guaranteed, and quantities needed may vary significantly throughout the year.

Work Details
The eyeglasses must be durable plastic frames with no metal spine in the temple pieces or any part of the frames. Plastic frame color choices are preferred, and frames should be available in extra small, small, medium, and large sizes for both men and women.
Each pair of glasses shall come with a case containing no metal. Lenses are to be at least CR39 plastic lenses with scratch-resistant coating, including single vision and bifocals. Spare parts will be provided upon request.
The awarded offeror is preferred to buy back unclaimed, refused, or erroneously prescribed eyeglasses at 75% of the original cost. Completed eyeglass orders will be sent to any one of the nineteen individual correctional facilities throughout the state of Colorado.
Each pair of eyeglasses shall be individually identified with information furnished by the ordering facility including the inmate name, CDOC #, and copy of the prescription.

Period of Performance
The initial term of the awarded Contract is anticipated to be one year and may be renewed for up to four additional one-year periods at the sole discretion of the CDOC. The total duration of the Contract is not anticipated to exceed five years.

Place of Performance
Prescription eyeglasses will be shipped to offenders housed at any of the nineteen specified CDOC facilities located throughout Colorado.
