Background
The County of Solano, Department of Health and Social Services (H&SS), Child Welfare Services (CWS) intends to secure a contract with a qualified proposer to provide Transitional Housing Program-Plus (THP-Plus) services to young adults in Solano County.
THP-Plus is designed for young adults who have exited foster care on or after their 18th birthday and are not yet 25 years old. The program aims to ensure a safe living environment for these individuals, helping them develop life skills related to independence, prevent homelessness, and achieve outcomes outlined in their Transitional Independent Living Plans (TILP).
Additionally, the County seeks to reduce barriers and increase access to housing programs through the Housing Navigation and Maintenance Program (HNMP), which supports youth in stabilizing housing while addressing issues contributing to instability.
Work Details
The contractor will provide the following services under THP-Plus: affordable housing and comprehensive supportive services for up to 36 months for former foster care and probation youth aged 18-25. Services include case management, educational support, employment assistance, crisis intervention, and development of Transitional Independent Living Plans tailored to individual needs.
The program will prioritize participants who are former Solano County dependents or wards. The HNMP will involve providing case management services that include acting as a Housing Navigator to assist youth with locating housing, completing applications, overcoming barriers to housing attainment, maintaining housing stability, connecting youth with community resources, and educating them on tenant rights and responsibilities.
The program aims to serve no less than 20 concurrent participants at maximum capacity.
Period of Performance
The contract is expected to commence on July 1, 2025, and will terminate on June 30, 2028. The total estimated cost for the three-year contract is not to exceed $3,565,486.
