Background
The County of San Mateo, located on the San Francisco Peninsula, serves a diverse population of nearly 800,000 residents across 20 incorporated cities and unincorporated areas.
The mission of San Mateo County Health (County Health) is to protect the health of all individuals in the county by preventing communicable diseases, providing targeted healthcare services, and fostering healthy communities. The County Health seeks proposals from qualified organizations to lead the planning and development of community collaboratives that incorporate community voices and diverse perspectives into health system improvements.
Work Details
The contractor will be responsible for organizing, facilitating, and leading the development of community collaboratives as part of the Community Health Improvement Plan (CHIP). Key tasks include:
1) Hiring and training staff for effective facilitation;
2) Facilitating at least five CHIP Collaborative meetings annually;
3) Identifying and recruiting diverse stakeholders;
4) Engaging with community members for feedback on health priorities;
5) Distributing stipends to community co-leads and participants;
6) Developing metrics to evaluate outcomes;
7) Managing logistics for meetings and events;
8) Sharing budget information with the CHIP Steering Committee annually.
The budget for this project is capped at $375,000 over two years, with funding for the second year contingent upon securing external funds.
Period of Performance
The contract will have a term of two years, starting from June 15, 2026.

The services will be performed within San Mateo County.
Bidder Requirements
