Description

The purpose of this Request for Proposal (RFP) is to obtain proposals from qualified Proposers to provide the Department of Consumer Affairs (DCA), Board of Pharmacy (Board), with the collection and evaluation of medication error reports consistent with the provisions outlined in Business and Professions Code (BPC) section 4113.1. For purposes of this RFP, medication error includes any variation from a prescription drug order not authorized by the prescriber, including, but not limited to, errors involving the wrong drug, the wrong dose, the wrong patient, the wrong directions, the wrong preparation, or the wrong route of administration. A medication error does not include any variation that is corrected prior to dispensing to the patient or patient s agent or any variation allowed by law.
Background
The Department of Consumer Affairs (DCA), Board of Pharmacy, is seeking proposals to collect and evaluate medication error reports as mandated by the Business and Professions Code (BPC) section 4113.1. This initiative aims to enhance patient safety by addressing medication errors, which are defined as any unauthorized variations from a prescription drug order.

The DCA's mission includes ensuring the safety and efficacy of medications dispensed in California. This contract is a critical step towards achieving that goal.

Work Details
The contractor will be responsible for:
1. Developing or utilizing an existing medication error reporting system for community pharmacies to report errors as required by BPC section 4113.1.
2. Collecting data elements including pharmacy identifiers, types of pharmacies, dates of incidents, types of events, patient demographics, and details about the medications involved.
3. Analyzing reported errors to identify patterns and trends within the first six months and on a semi-annual basis thereafter.
4. Providing narrative descriptions of findings and recommendations to reduce patient harm annually.
5. Submitting reports in specified formats (Microsoft Word for narratives and Excel for data) as directed by the Board.

Period of Performance
The anticipated contract term is from December 1, 2024, through November 30, 2027.

Place of Performance
The contract will be performed in California, specifically involving community pharmacies licensed by the Board.
