Rfi - Safety Net Bridge Program

Background
The County of San Diego is issuing this Request for Information (RFI) to gather input from interested parties regarding a Safety Net Bridge Program. This initiative aims to address potential gaps in CalFresh and Medi-Cal benefits for residents due to recent federal and State regulations outlined in the H.R.1 bill. These changes may lead to decreased access to essential services such as food, primary care, and prescription medications. The County is considering contracting with Community Health Centers (CHCs) to provide integrated primary care medical services alongside social services.

Work Details
The CHCs will be expected to deliver primary care medical services that are co-located with social services and access to prescription medications. Services will include:
- Primary care through telehealth modalities (video and telephone visits)
- Mobile Medical Units (MMUs) providing routine and preventive care at scheduled community locations, including health fairs.

The RFI seeks input on various aspects of service delivery, including effective coordination of medical services with social services, barriers to telehealth, and strategies for pairing food distribution with primary care.
