Background
Inland Empire Health Plan (IEHP) is a not-for-profit health plan established on July 26, 1994, serving over 1.5 million members in San Bernardino and Riverside counties.
The organization aims to streamline patient transitions across the care continuum, including discharge planning and coordination with community-based providers.

Work Details
IEHP seeks a software platform that facilitates the following:
- Provider Network Management: Maintain and update a directory of post-acute care providers (SNFs, HHAs, IRFs, LTACHs).
- Referral Management: Automated workflows for sending, tracking, and confirming referrals.
- Patient Engagement: Secure communication tools for patients and families to review options and participate in care decisions.
- Data Analytics & Reporting: Real-time dashboards on referral patterns, provider performance, and patient outcomes.
- Integration Capabilities: Interoperability with existing EHR systems (e.g., Epic, Cerner) via HL7/FHIR standards.
Compliance Requirements include adherence to HIPAA regulations and maintaining necessary certifications (e.g., SOC 2, HITRUST).
Performance expectations involve system availability of at least 99.9% uptime and timely response to referral transmissions.

Period of Performance
The RFI is issued on March 6, 2026, with responses due by April 9, 2026.

Place of Performance
The contract will be performed in Rancho Cucamonga, CA.

Bidder Requirements
Vendors must certify no conflicts of interest with IEHP staff or governing board.
They must also provide documentation for non-debarment status and maintain all necessary licenses and certifications.
