IFB #SD24-00035 Hearing Aid Services and Devices

Background
The California Department of Corrections and Rehabilitation (CDCR) and the California Correctional Health Care Services (CCHCS) are inviting responses to this Invitation for Bid (IFB) titled Hearing Aid Services and Devices, Bid Number SD24-00035.
This contract aims to provide necessary hearing aid services and devices to patients referred from various CDCR/CCHCS institutions, ensuring that health care services are accessible and effective for individuals in custody.

Work Details
The contractor will provide on-site Hearing Aid Services and Devices as needed, including:
1. Audiometry Screening (HCPCS Code V5008) at a rate cap of $20.00 per patient.
2. Pure Tone Audiometry (HCPCS Code X4501) at a rate cap of $40.00 per patient.
3. Speech Recognition Test (HCPCS Code X4500) at a rate cap of $70.00 per patient.
4. Follow-up Evaluation at a rate cap of $30.00 per patient.
5. Daily Clinic services at a rate cap of $80.00 per day.
6. Ear Mold provision at a rate cap of $90.00 per ear.
7. Manufacture Hearing Aid provision at a rate cap of $1,000.00 per device.
8. Major Hearing Aid Repair at a rate cap of $240.00 per device.
The total basis of award is set at $1,570.00 for the combination of services and goods provided under this contract.

Period of Performance
The contract will be executed for an initial term of 24 months with an option to extend for an additional 12 months, starting from the proposed start date of September 19, 2025.

Place of Performance
Services will be performed at various CDCR/CCHCS institutions across California as specified in Exhibit G – List of Participating CDCR Institutions.

Bidder Requirements
Bidders must comply with specific licensing requirements, including valid licensure or board certification without pending accusations in their medical specialty area.
Additionally, bidders must submit all required documents listed in Attachment 1 – Required Attachments Checklist, including proof of training and certifications relevant to the services provided.
