EXT HSA | RFP 1166 Support at Home

Background
The San Francisco Human Services Agency (SFHSA) and the Department of Disability and Aging Services (DAS) are seeking proposals from nonprofit organizations to provide a home care voucher program for older adults and adults with disabilities facing unmet home care needs due to affordability challenges in San Francisco.

The program aims to improve health, safety, and quality of life outcomes for individuals who cannot access other subsidized home care services. The Support at Home program has been operational since 2016 and currently has a waitlist, indicating a high demand for such services.

Work Details
The selected contractor will implement a home care voucher program that includes:
1. Domestic services: grocery shopping, meal preparation, cleaning, laundry, and general housework.
2. Non-medical personal services: bathing, oral hygiene, grooming, dressing, transfers, bowel and bladder care.
3. Accompaniment services: assistance to and from medical appointments.
4. A sliding scale payment system based on client needs and financial capacity.
5. A maximum of 15 hours of funded service per week per client with potential exceptions approved by DAS.
6. Client choice between agency-managed or independent provider services.
7. Minimum wage for providers set at $20.25 per hour as of July 1, 2024.

Period of Performance
The contract is anticipated to last four years from July 1, 2025, to June 30, 2029, contingent upon annual funding availability and satisfactory performance.

Place of Performance
The contract will be performed within the City and County of San Francisco.
