Background
The County of Santa Cruz invites proposals from licensed, insured, bonded, and certified contractors to provide Enhanced Care Management services. The county aims to improve access to services through a centralized system that integrates various data sources for better care coordination as part of the CalAIM initiative.

Work Details
The contractor will implement a comprehensive system of record that supports seamless collaboration across departments and external providers. Key requirements include:

- Interoperability with existing systems
- Closed-loop referral workflows
- Integration with Health Information Exchanges (HIEs)
- Support for automated eligibility checks
- Consent management capabilities
- Robust analytics and reporting tools

The system must comply with HIPAA and other privacy regulations while facilitating real-time data exchange and tracking client participation across multiple programs.
