Background
The Department of Public Health (DPH) of the City and County of San Francisco is seeking proposals for Health Information Exchange (HIE) services to comply with the Data Exchange Framework mandated by California Assembly Bill 133 (AB 133).
The goal is to enable DPH to exchange health information with its largest insurance provider, the San Francisco Health Plan (SFHP), and other Covered Entities as defined by HIPAA.
The successful proposer must be a Qualified Health Information Organization (QHIO) as defined in AB 133.
Work Details
The contractor will provide HIE services including: onboarding and ongoing services between DPH’s EHR system and SFHP, supporting data transfer and compliance with regulatory requirements, developing and using CCDA and FHIR APIs, delivering digital workflows for querying healthcare admissions, functioning as a broker for ADT messages using HL7 standards, managing patient/client identity and demographics, enabling electronic consent management, providing eligibility information for SFHP members via FHIR resources, offering portal access for the Chief Medical Examiner, and ensuring compliance with future state or federal requirements.
The onboarding phase must be completed within ninety days of contract certification.
Optional services include providing access to encounter ADT feeds for public health monitoring, a user interface for public health surveillance, vaccination surveillance capabilities, and integration with EMS providers' pre-hospital care records.
Period of Performance
The contract will have an initial term of three years with seven one-year options to extend at the City’s discretion.
The estimated start date is March 15, 2026.

Services will be performed in San Francisco, California.
Bidder Requirements
Proposers must have at least ten years of experience in providing HIE services to healthcare entities.
They must also be a Qualified Health Information Organization (QHIO) as defined in AB 133.
