No.54-0415, AZ Br. No. 957 from R153.9 to R154.7 on the California side and within Arizona from PM 0.0 to 0.6. A more detailed description of the Project is contained in Appendix A. The Original shall be identified on its front cover, in the upper right-hand comer as ;Original; All SOQs shall be received by 3 p.m. Pacific Time on the day specified in Section 2.- Procurement Schedule, unless modified by Addenda. For hand-delivered SOQ submittals to Sacramento, call Charlie Sparkman at (916) 709-9686 to prearrange the day and time for SOQ hand delivery. Please refer to the RFQ for goal requirement and attachments for licensing requirements for this contract. Unfortunately, the Department does not have a plan holders list of available proposers, nor does it have a way of identifying companies or entities that may have downloaded the project documents for teaming purposes. It is the responsibility of the consultants/subs/vendors to identify teams that may be submitting an Statement of Qualifications. The Department will allow the use of text with font size smaller than 12-point for only charts, tables, exhibits, and other illustrative and graphical information. Addendum 1 - The purpose of this addendum is for the Department to respond to concerns submitted by proposers. Addendum 2 - Questions and Answers Addendum 3 - The purpose of this Addendum is to provide a revise Form G as indicated in Question 1 below and to provide a revised table as listed in section 2 Procurement Schedule of the RFQ. Addendum 4 - The purpose of this Addendum is for the Department to provide responses to concerns submitted by proposers and a copy of the SHPO agreement included in this addendum.
