Background
Alameda County Health (AC Health) is seeking to provide medical respite services for individuals at risk of or currently experiencing homelessness. The goal of this contract is to operate the Fairmont Medical Respite site, which serves as a recuperative care center. This ensures that individuals have access to medically appropriate housing options upon exit. The County intends to award a one-year contract with the option to renew for four additional years to the most responsible bidder whose proposal meets the requirements outlined in the RFP.

Work Details
The contractor will be responsible for operating the Fairmont Medical Respite site located at 2055 Fairmont Dr, San Leandro, CA 94578, which currently has 34 private units. Of these, 15 units are designated for medical respite interim housing for individuals who are too ill or frail to recover on the street but do not require hospitalization. The remaining 19 units serve as Navigation Center interim housing.

Services include:
1. Medical Respite/Recuperative Care services including nursing and caregiver support.
2. Daily interaction with clients in a safe environment.
3. On-site nursing support Monday-Friday and on-call coverage 24/7.
4. Transportation assistance for medical appointments.
5. Coordination with housing providers for permanent housing transitions.
6. Provision of meals and laundry services.
7. Maintenance of client safety and security through regular checks and documentation.
8. Compliance with Alameda County’s Emergency Shelter Standards.

Period of Performance
The contract is set to begin on July 1, 2025, and will last for one year with an option to renew for up to four additional years.
