The purpose of the public TAKEheart website is to serve as a current, evidence-based resource to increase national awareness of underutilization of Cardiac rehabilitation, demonstrate the value and lifesaving benefits of Cardiac rehabilitation, and promote existing resources available on Cardiac rehabilitation referral, enrollment and retention to eligible populations. Cardiac rehabilitation science is continually being updated and an easily accessible website will be a valuable resource for CR stakeholders, including: hospital performance improvement teams; quality improvement organizations (QIOs) and QI teams; healthcare providers (cardiologists, primary care providers, nurses); insurers and patients. The private portion of the website serves as a collaborative working space for intervention hospitals to share documents, comment on questions and share lessons learned.
