MEDICAL HEALTH SERVICES PROVIDED FOR ELIGIBLE NATIVE AMERICAN BENEFICIARIES IN THE METRO MINNEAPOLIS AREA. INCLUDES THE PROVISION OF URBAN HEALTH SERVICES, TELE-BEHAVIORAL SERVICES, AND MEDICAL CARE, AS WELL AS IMPLEMENTATION OF A MINI COG PILOT DEMENTIA GRANT. THE CONTRACT ALSO SUPPORTS PROGRAMS SUCH AS THE URBAN/ALCOHOL PROGRAM AND DISBURSEMENT OF RECURRING AND NON-RECURRING FUNDING FOR THESE SERVICES.
