Government Description
EL CENTRO CBOC - PRIMARY CARE SERVICES - VASDHS. PROVIDES COMPREHENSIVE PRIMARY CARE SERVICES FOR VETERANS AT THE EL CENTRO COMMUNITY-BASED OUTPATIENT CLINIC (CBOC), INCLUDING PREVENTIVE CARE, DIAGNOSIS, TREATMENT, AND REFERRALS FOR SPECIALTY CARE. SERVICES ARE DELIVERED BY A PATIENT ALIGNED CARE TEAM (PACT) AND INCLUDE INTEGRATED MENTAL HEALTH CARE THROUGH PRIMARY CARE-MENTAL HEALTH INTEGRATION (PCMHI). ALL CARE IS PERFORMED IN ACCORDANCE WITH VHA PERFORMANCE AND QUALITY CRITERIA AND VA POLICIES WITHIN EL CENTRO, CALIFORNIA.
