Government Description
Hchv geriatric services provides emergency housing and supportive services for homeless veterans in northern California. The program offers secure housing, three daily nutritious meals, case management and care coordination, vocational and benefits services, enhancement of independent living skills, and support in finding permanent or transitional housing. Services are prioritized for homeless veterans eligible for VA health care, particularly those transitioning from chronic street homelessness, being discharged from institutions, or who have recently become homeless.
