5 year IDIQ for HCHV services vasnhcs reno. The community based Health Care for Homeless Veterans (HCHV) program provides short-term residential care and treatment to eligible veterans needing immediate housing placement as they seek permanent housing and/or additional care and services. Services include safe, secure housing in community-based settings, supportive case management, assistance with permanent housing searches, vocational and benefits support, mental health and substance use disorder treatment, and life skills development, all with 24/7 coverage. The program prioritizes veterans transitioning from street homelessness or institutions, offers flexible admission practices, and emphasizes individualized care planning and engagement to achieve stable, permanent housing.
