IDIQ for HCHV services for male veterans vasnhcs reno. The community based Health Care for Homeless Veterans (HCHV) cers program provides short-term residential care and treatment to eligible male veterans in reno, NV, offering immediate housing placement, supportive services, and care coordination as they seek permanent housing and/or additional services. The program includes safe, secure housing, case management, mental health and substance use disorder treatment, vocational and benefits assistance, independent living skills training, and access to three daily meals, with a focus on reducing barriers to admission and facilitating engagement in recovery and housing stabilization. Services provided 24/7 for up to 20 veterans at a time, prioritizing those transitioning from street homelessness or institutional discharge, and emphasize individualized care planning, rapid placement, and ongoing collaboration with VA resources.
