Government Description
CBOC kirksville, MO 589A4. This award provides comprehensive primary care and mental health services to veterans in kirksville, MO and surrounding counties through a Community Based Outpatient Clinic (CBOC), including both on-site and telehealth care, as well as support services such as pharmacy, laboratory, and x-ray. The contractor is responsible for staffing patient aligned care teams (PACTS) with qualified primary care providers, registered nurse care managers, clinical associates, and administrative associates, and for ensuring the provision of discipline-specific services such as clinical pharmacy, social work, nutrition, and mental health integration. Additional services include general and specialty mental health care, telehealth clinical support, ancillary services (such as phlebotomy), and outreach for suicide prevention and homeless veterans, all delivered in accordance with VA standards and requirements.
