Government Description
Provide temporary housing for homeless veterans. Under the Health Care for Homeless Veterans (HCHV) program at the lexington, KY VA Medical Center (VAMC), the contractor develops a comprehensive Individual Service Plan (ISP) for each veteran, including permanent housing goals, and delivers daily structured group activities and individual instruction. The contractor assists with housing searches, maintains daily progress reports, conducts bi-weekly meetings with VA staff, and provides support for personal hygiene, community interaction, and orientation for new residents. Additional responsibilities include developing bilingual house rules, implementing pest management, enrolling staff in Homeless Management Information System (HMIS) training, ensuring compliance with security and drug testing protocols, and meeting federal and state guidelines for residential programming.
