Grant Program Funds Requested in the Application: $11,929,766.
Purpose: Louisiana has consistently ranked poorly in comparison to the rest of the country on overall maternal and child health outcomes and social determinants of health. The Louisiana MIECHV (LA MIECHV) grant project proposes to sustain and strengthen effective, evidence-based home visiting services via implementation of Nurse-Family Partnership (NFP) and Parents as Teachers (PAT) in communities identified as at-risk in the 2020 Needs Assessment.
LA MIECHV will use the additional funding available with this award to bolster our efforts to retain and strengthen the home visiting workforce. The additional funds will allow us to offer competitive salary rates for open positions, increase hazard pay for all staff, and maintain the presence of services in parishes no longer funded by Title V.
Goal(s) and Objectives:
Goal 1: LA MIECHV will sustain evidence-based home visiting and improve maternal, child and family health and well-being in Louisiana.
- Objective 1.1: For the duration of the project period, continue to implement Nurse-Family Partnership (NFP) in 28 MIECHV-funded parishes across LDH administrative regions 2-9 identified as at-risk in the 2020 LA MIECHV Needs Assessment and 2021 Early Childhood Risk and Reach Report.
- Objective 1.2: For the duration of the project period, continue to implement Parents as Teachers (PAT) in 23 MIECHV-funded parishes across LDH administrative regions 1, 6, 7, 8, and 9 identified as at-risk in the 2020 LA MIECHV Needs Assessment and 2021 Early Childhood Risk and Reach Report.
- Objective 1.3: For the duration of the project period, provide infrastructure to support the effective implementation of evidence-based home visiting via continuation of contracts for key infrastructure staff (10.0 FTE) and for Find Help (CIRS).
Goal 2: LA MIECHV will strengthen the implementation of MIECHV evidence-based home visiting through prioritization of parent leadership and improved coordination with key early childhood system partners.
- Objective 2.1: By September 29, 2025, engage parent leaders on each of the 18 regional home visiting teams via implementation of activities as outlined in Stages 1-3 of the HV COIIN 2.0 Parent Leadership Toolkit.
- Objective 2.2: By September 29, 2025, establish quarterly meetings with all 6 Healthy Louisiana plans and initiate MOU discussions with United Healthcare, Humana, and Healthy Blue.
Methodology: Evidence-based models: NFP and PAT.
Communities served and target population group(s): LA MIECHV prioritizes serving low-income families in 40 parishes identified as at-risk in the 2020 Needs Assessment and 2021 Early Childhood Risk and Reach Report. To be eligible for NFP or PAT services through LA MIECHV, families must also be eligible for Medicaid, SNAP, WIC, TANF, or SSI at enrollment. Additional subpopulations are represented in LA MIECHV's client base, but current eligibility criteria does not specifically select for them.
Proposed caseload of MIECHV family slots: FY 23: 1,762; FY 24: 1,762. Current caseload of MIECHV family slots: 1,776.
Appropriate linkages and referral networks to other community resources and supports, as well as comprehensive statewide early childhood systems are ensured via the collective work of statewide home visiting staff and state-level leadership. Key activities include continued participation in cross-system advisory and ad-hoc workgroups, provision of dedicated staff time to spearhead partnership and coordination efforts, progress toward statewide implementation of a central intake system, and continued co-leadership of the community advisory and action teams in each of the nine LDH administrative regions.
