Purpose:
MIECHV funds will provide Indiana resources to continue home visiting and improve outcomes for at-risk families through Healthy Families Indiana (HFI) and Nurse-Family Partnership (NFP). As of March 31, 2023, Indiana has 1,192 families enrolled in MIECHV-funded home visiting. Clients are often characterized by risk factors including: mental illness, substance abuse, low educational attainment, history of interpersonal violence, single parenting, and limited access to health care. Home visiting will address high-risk, low-income families in need of services through education, intervention, and referrals to support services.

Goals and Objectives:
Indiana’s MIECHV vision is to improve health and development outcomes for children and families who are at risk of adverse health and development outcomes through achievement of the following goals:
1) Provide home visiting services to pregnant people and families with young children residing in Indiana who have lower incomes and are at higher risk of adverse health and developmental outcomes to improve their health and well-being;
2) Develop a system of statewide coordinated services statewide of existing and newly developed home visiting programs in order to provide need-based, targeted, and unduplicated services and locally-coordinated referrals to all eligible children, mothers, and families;
3) Coordinate necessary services outside of home visiting programs to address needs of participants.

These goals will be achieved through the following objectives:
Continue program implementation serving at least 1580 new and continuing families.
- By 9/30/25, HFI will serve 905 MIECHV-funded families in Elkhart, Lake, LaPorte, Marion, Scott, St. Joseph, Bartholomew, Boone, Clark, Dearborn, Fayette, Floyd, Franklin, Jackson, Jefferson, Jennings, Montgomery, Ohio, Owen, Putnam, Ripley, Rush, Switzerland, and Union counties.
- By 9/30/2025, NFP will serve at least 675 MIECHV-funded families in Elkhart, LaPorte, Marion, and St. Joseph counties.

Inform organizations in Indiana regarding referral coordination & expansion of home visiting services.
- By 9/30/25, 100% of facilitated meetings of Indiana Early Childhood Collaborative (INECC) with key representatives from state level social service departments will include an update regarding MIECHV-funded home visiting and coordinated state evaluation (CSE) activities.
- By 9/30/25, collaborate with Indiana state agencies administering or funding home visiting services or home visiting service supports to develop an alignment of goals and strategies for implementation and support of home visiting programs across Indiana.

Indiana will increase the number of referrals to additional services beyond home visiting when a participant need is identified for positive screen for depression.
- By 9/30/2025, Indiana will reduce missing data for the completed depression referrals measure to less than 10%.

Methodology:
HFI and NFP will provide MIECHV-funded services to at least 1580 families during the project period in Elkhart, Lake, LaPorte, Marion, Scott, St. Joseph, Bartholomew, Boone, Clark, Dearborn, Fayette, Floyd, Franklin, Jackson, Jefferson, Jennings, Montgomery, Ohio, Owen, Putnam, Ripley, Rush, Switzerland, and Union counties. Program outcomes will be evaluated quarterly and reported annually. Quality assurance and improvement will ensure voluntary participants receive model adherent services and referrals.
