The South Carolina Ryan White Part B (RWB) Program, including the AIDS Drug Assistance Program (ADAP) administered by the South Carolina Department of Public Health (DPH), respectfully requests emergency relief funding under HRSA-26-012 to sustain uninterrupted access to essential medication and insurance assistance for vulnerable people with HIV (PWH) in South Carolina (SC).

This request is strategically designed to avert service disruptions, including the imposition of a waiting list or major cutbacks, during a period of decreased funding and increasing costs.

ADAP services for PWH in SC are available through direct dispensing, insurance assistance, and Medicare Part D assistance.

Recent decreases in available funding and increased expenditures have accumulated to a budget shortfall across the Ryan White Part B projected FY026 budget.

Contributing factors include:

1) Increasing clients served due to growing client demand tied to increased incidence and prevalence, rapid linkage and reengagement, and retention in care efforts;

2) Dramatic decrease in rebates in FY2025; and

3) Gradually increasing ADAP costs with additional increases anticipated in FY2026.

Per the SC EPI Profile (2023), during the calendar year of 2017, based on the CDC HIV/AIDS Surveillance Report, SC ranked 11th among states, the District of Columbia, and U.S. dependent areas with an AIDS case rate of 8.6 per 100,000 population.

The epidemic is continuing to grow with an average of 67 cases of HIV infection reported each month during 2022.

The incidence rate in S.C. for 2022 is 15.3 per 100,000 population.

As of December 31, 2022, there were an estimated 20,016 S.C. Carolina residents living with diagnosed HIV infection (including AIDS).

The requested $7 million of ADAP ERF will be used for ADAP expenses for the direct dispensing program and insurance assistance program.

These funds will provide ADAP medication sustainability (no waiting list) to those who routinely apply for ADAP services, as SC implements 340B-generating improvements to sustain ADAP.
