Grant Description
Geriatrics Workforce Enhancement Program - Saint Louis University Gateway Geriatric Education Center requests funding for the Geriatrics Workforce Enhancement Program (GWEP), including Alzheimer’s Disease and Related Disorders (ADRD) education, to meet the rising needs for primary care health providers and delivery systems to provide competent care for physical and cognitive health of older adults and their care partners in Missouri.
Our state continues to encounter healthcare workforce shortages in 99% of the counties and increasing numbers of older adults that exceed population growth, many of whom experience poverty and social and health disparities.
Building on GWEP’s age- and dementia-friendly initiatives in which six partners received age-friendly health system recognition, 10,000+ individuals were trained in geriatric assessment who completed 16,000+ rapid geriatric assessments, we aim to increase competencies for students, residents, fellows, faculty, and providers in geriatric screening, assessment, and intervention by creating age- and dementia-friendly organizations, using evidence-based, interprofessional interventions to transform practices in caring for older adults and families to ensure knowledge and skills for future and current professionals.
The project reaches all 8 Missouri US congressional districts and a region of southern Illinois targeting medically underserved and rural areas.
We will increase accessible education and care, improve population health and quality, and integrate system-wide age- and dementia-friendly health care transformations across these areas.
Goals and objectives include:
1) Through reciprocal partnerships with Missouri universities, primary care sites and delivery systems (including post-acute and long-term care systems (PALTC), and community organizations, we will transform clinical training environments into integrated geriatrics and primary care/delivery systems that are age- and dementia friendly by:
A) Expanding age- and dementia-friendly care in medically underserved and rural sites with support for education and training, developing integrated geriatric and behavioral health services, including geriatric clinics, GWEP-developed software for assessment, Medicare annual wellness visits, and dementia care interventions (cognitive stimulation therapy);
B) Addressing PALTC workforce needs through education and training, including: develop registered apprenticeship program model CNA apprenticeship and certificate programs in advanced geriatric specialties for supportive and direct care staff, promotion of PALTC careers for nursing students, and support for PALTC facilities to achieve AFHS recognition;
2) Provide interprofessional geriatrics clinical training and education through clinical rotations and didactic education to students, residents, fellows, faculty, and preceptors in primary care sites/delivery systems to support graduates practicing in these sites post-graduation through multiple educational strategies, provide clinical interprofessional geriatric, dementia, and caregiver education to strengthen age- and dementia-friendly knowledge and skills for those in training and practice through didactic, experiential, and clinical experiences in underserved and rural sites and settings (acute, ambulatory, and PALTC sites, telehealth, and mobile health van services).
3) Establish and maintain education and training programs in underserved and rural primary care sites/delivery systems to provide primary care and supportive and direct care staff with knowledge and skills to improve the care of older adults, including persons with dementia, via innovative technology strategies and methods such as accessible and culturally and linguistically appropriate training and technology, using dementia 360 framework to guide dementia care interventions, and address education and service needs of older adults and caregivers experiencing behavioral health challenges—dementia, loneliness and social isolation, and caregiver well-being.
