Grant Description
New Hampshire High Impact HIV Prevention and Surveillance Program - The New Hampshire (NH) Department of Health and Human Services (DHHS), Division of Public Health Services (DPHS) will provide comprehensive and integrated HIV surveillance and prevention activities to prevent new infections and improve health outcomes for persons living with HIV infection.
This includes achieving and sustaining viral suppression and reducing health disparities in accordance with national prevention goals, HIV care continuum and CDC’s High Impact Prevention (HIP) and whole-person approach by using quality, timely and complete surveillance and prevention program data to guide HIV prevention efforts.
NH DPHS will place particular focus on syndemic prevention efforts for populations at greatest risk of acquiring HIV and use enhanced surveillance efforts to monitor impact.
