Grant Description
National HIV Behavioral Surveillance (NHBS) - The objective of the National HIV Behavioral Surveillance System (NHBS) is to investigate HIV risk, testing, and prevention services in order to design and evaluate HIV prevention efforts in New York City (NYC) and throughout the United States.

The NYC Department of Health and Mental Hygiene (DOHMH) proposes to conduct an ongoing, cyclical, cross-sectional research study of HIV risk among populations that historically have been placed at high risk for HIV to accomplish this objective. NHBS would include qualitative research, quantitative survey research, and HIV testing of three populations at increased risk between 2022 and 2026: men who have sex with men (MSM), people who inject drugs (PWID), and heterosexually active adults at increased risk for HIV (HET).

DOHMH proposes to study PWID in 2022 and 2024, MSM in 2023 and 2026, and HET in 2025. The NHBS quantitative survey would include questions in four domains: demographics, HIV risk behaviors, HIV testing, and the use of HIV prevention services. DOHMH will enroll members of the target population through venue-based (MSM cycles) and respondent-driven sampling (PWID and HET cycles). DOHMH will complete NHBS surveys with a minimum of 500 participants in each cycle, for a total minimum of 2,500 surveys over the five-year project period.

Additionally, DOHMH proposes to study transgender women (TRANS) and women who exchange sex (NHBS-WES). For these supplemental cycles, DOHMH will enroll 300 participants each using respondent-driven sampling. DOHMH will conduct anonymous bloodborne HIV testing to estimate HIV prevalence of each population. Additionally, if funding is available, DOHMH will conduct anonymous testing for hepatitis C during PWID, TRANS, and WES cycles and anonymous testing for gonorrhea and chlamydia in MSM, HET, TRANS, and WES cycles.

Throughout the NHBS research study, DOHMH will collaborate with community partners including the local HIV prevention planning group, HIV Health and Human Services Planning Council of New York, and federally funded community-based organizations. DOHMH will also collaborate with local researchers with knowledge of the target populations.

Finally, DOHMH will conduct evaluations of the NHBS system to refine the methods of HIV behavioral surveillance and will conduct analyses of NHBS data to describe the sampled populations and to investigate specific research questions on HIV risk.

The number of people impacted by DOHMH's involvement in NHBS will be substantial. We estimate that we will be able to directly serve approximately 2,500 adults through their involvement in the HIV and STI screenings offered to core NHBS participants. In addition, we expect to directly serve at least 300 TRANS participants and 300 WES participants. This estimate is based on the target populations for the various cycles of NHBS and NYC DOHMH's excellent history of exceeding recruitment targets in past NHBS cycles.

In terms of the impact of this work, DOHMH will indirectly serve the HIV prevention needs of a much larger population of New Yorkers. Study findings will inform program planning and resource allocation for nearly 760,000 heterosexually active adults at increased risk of HIV; 188,000 MSM; 103,000 PWID; and 17,000 transgender women in NYC, for a combined impact on more than 1,068,000 people. This estimate is based on a variety of data sources, including NYC surveillance data and health statistics.

Overall, we are confident that the involvement of NYC DOHMH in the NHBS will make a significant impact on HIV prevention in NYC.
