Grant Description
West Alabama Cardiovascular Health (WACH) Program - The West Alabama Cardiovascular Health (WACH) Program is designed to reduce the risk of cardiovascular disease events (heart attacks and stroke) in a nine-county region of West Alabama.
Alabama has some of the highest rates of hypertension in the country, ranked at 47 with 42.7% of the population having a diagnosis, and with Alabamians having a 10-percentage point higher hypertension rate than the national average.
WACH will work with state and community partners to: 1) develop local cardiovascular learning communities, 2) increase the use of Alabama's Health Information Exchange (HIE), 3) use the HIE to monitor CVD risk in the targeted communities, 4) deploy community health workers (CHWs) to educate and connect residents to social services, and 5) implement programming to prevent CVD in high-risk communities in West Alabama.
