Grant Description
The National Cardiovascular Health Program - The Indiana Department of Health (IDOH) Division of Chronic Disease, Primary Care and Rural Health is applying for the Centers for Disease Control and Prevention (CDC) CDC-RFA-DP23-0004 than National Cardiovascular Health Program. This funding opportunity will support a coordinated effort across IDOH program areas to improve the health of Indiana's residents in the communities.
IDOH serves all citizens and communities within Indiana. Activities proposed by IDOH will target those areas in Indiana that are disproportionately impacted by high rates of cardiovascular disease, hypertension, and cholesterol. To achieve the outcomes indicated within the Notice of Funding Opportunity, Indiana will partner with health systems located within areas of cardiovascular high risk in order to increase the use of evidence-based practices such as team-based care, patient registries to identify patients with high blood pressure and high cholesterol.
The patients' social determinants of health will be utilized to refer patients to social services or other community support programs that are needed. IDOH will also work directly with the health systems and community-based organizations to increase the utilization of community health workers for hypertension and high cholesterol. Sustainability activities for the CHWs will be addressed at the state-level for policy development, Medicaid reimbursement, and a process for education and certification.
IDOH will establish a cardiovascular learning collaborative utilizing the IDOH experience with learning collaboratives with the grant's funded health system. The learning collaborative will allow IDOH to share information to all health systems, to enable peer-to-peer learning, apply the strategy of using PDSA cycles for quality improvement, and the planned care model for these systems.
