Grant Description
The National Cardiovascular Health Program - The New Jersey Department of Health (NJDOH) will implement local-level, evidence-based strategies to address cardiovascular disease. The focus will be on reducing risks for hypertension and high cholesterol and mitigating social determinants of health barriers.
NJDOH will target strategies in Southern NJ and conduct geographic information system (GIS) mapping to identify census areas within Atlantic, Cape May, Cumberland, and Salem counties.
NJDOH will collaborate with partners and existing coalitions to create a Southern Regional Learning Collaborative. This collaborative will consist of partners operating within the target communities, including health systems, Federally Qualified Health Center (FQHC) sites, local health departments, community and faith-based organizations, practitioners, and community members.
The learning collaborative will include a feedback loop, allowing information and recommendations to flow to all of the regional and state partners. This will ensure communication between the partners and filter the information to the state learning collaborative.
In addition, NJDOH will work with Health 2 Systems and FQHC sites to assess current practices for capturing and referring patients to support and social services.
