Grant Description
Alabama Cardiovascular Health Program - Cardiovascular disease is a devastating problem in Alabama. Heart disease is the leading cause of death, and stroke is fifth. Alabama presently has an adult diagnosed hypertension prevalence over 42 percent, ranking third in the nation. The hypertension prevalence is higher for Alabama's African American populations at 48.5 percent (27 percent of Alabama's total population).
Social determinants such as economic stability, healthcare access and quality, and social context affect these cardiovascular outcomes and efforts to reduce the likelihood of individuals developing these conditions. For example, 16 percent of Alabama's population lived in poverty during the 2020 Census. Nine Alabama counties do not have a hospital, and many counties have an area designated as a health professional shortage area.
The Alabama Department of Public Health will implement and evaluate culturally informed, evidence-based, and evidence-informed interventions and approaches to prevent, detect, control, and manage cardiovascular disease in Alabama. This will be achieved by creating a state and four local level heart disease and stroke learning collaboratives that address social services and support needs to improve health equity for populations and communities with the highest prevalence of CVD.
