Grant Description
The National Cardiovascular Health Program - Purpose:

Through Health Research, Inc., the New York State Department of Health (NYSDOH) requests funding to implement and evaluate evidence-based strategies to prevent and manage hypertension (HTN) and high cholesterol (HC). The COVID-19 pandemic devastated vulnerable populations with HTN and HC in NYS, especially those experiencing health inequities and disparities. With a long history of cardiovascular disease prevention leadership, NYSDOH is positioned to lead the NYS Cardiovascular Health Program.

NYSDOH, in collaboration with the Community Health Care Association of NYS, NY's Primary Care Association, will recruit Federally Qualified Health Centers (FQHCs) in the New York City and Central NY/Finger Lakes regions to implement and evaluate eight strategies:

1) Track and monitor HTN measures and identify disparities in control.
2) Identify social services and support needs (SSSNs) and refer patients to community services.
3) Support the use of HTN registries to identify disparities in care.
4) Adopt a team approach to identifying patients' SSSNs.
5) Convene a learning collaborative that will support FQHCs in removing barriers to accessing community services.
6) Refer patients with SSSNs to community programs.
7) Engage community health workers to address barriers to community services.
8) Promote the use of self-measured blood pressure monitoring with clinical support.

Outcomes:

NYSDOH will make measurable improvements in the following intermediate outcomes: improved blood pressure control among populations within partner health care and community settings, reduced disparities in hypertension control among populations within partner health care and community settings, and increased utilization of SSSNs among populations at highest risk for HTN and HC.
