Grant Description
Cardiovascular Health Program in New Hampshire - New Hampshire Department of Health & Human Services (NH DHHS) proposes to prevent cardiovascular disease through improving management of hypertension & high cholesterol in high-risk, priority populations. NH DHHS is prepared to implement all required strategies.
The planned outcomes are increased healthcare systems that use standardized quality measures to track hypertension control by race, ethnicity & other populations of focus and & the social services and support needs of priority populations; increased policies and protocols in place to use health information systems to support team-based care, increased social services & supports in the DHHS-led learning collaborative network, increased utilization of lifestyle change programs and social services & supports, and increased patients served by self-measured blood pressure programs.
NH will increase the proportion of priority populations with controlled hypertension & high cholesterol. The priority populations are adults over 35 years of age, living in rural areas, with low income & with diabetes/prediabetes.
Through extensive strategic partnerships, experience with all proposed strategies, and staff subject matter expertise related to the required strategies, NH DHHS has the capability to serve all populations and communities in NH that the program is proposing to work with.
