Overview

Grant Description
The National Cardiovascular Health Program - the South Dakota Department of Health (SD DOH), a state health department, is applying for funding under CDC-RFA-DP23-0004 - the National Cardiovascular Health Program (NCHP).

The proposed area of focus is the entire state of South Dakota, for a total statewide population reach of 895,376 people. The physical address of SD DOH is 600 East Capitol Avenue, Pierre, SD 57501-2536.

Funding South Dakota under this NCHP will enable the SD DOH's Heart Disease and Stroke Prevention Program (HDSPP) to implement and evaluate evidence-based and evidence-informed strategies to prevent and manage cardiovascular disease (CVD).

HDSPP will bolster its existing "Learning Collaborative" (LC) capacity, the SD Cardiovascular Collaborative, to reduce hypertension and high cholesterol in alignment with NCHP strategies as identified in DP23-0004.

Target populations will include South Dakotans impacted by high prevalence of CVD, potentially exacerbated by health inequities and disparities, including social determinants of health (SDOH).

Specifically, HDSPP will implement all DP23-0004 prevention and risk mitigation strategies, with an emphasis on reducing health disparities for priority populations (American Indian, Hispanic, Karen, low socioeconomic status, rural/frontier, and uninsured/underinsured).

The proposed project will be evaluated, and the data collected will be appropriately managed. Preliminary details surrounding the Evaluation and Performance Measurement Plan (EPMP) and the Data Management Plan (DMP) are described herein, with additional detail provided six (6) months post-award.

Under this project, SD DOH adopts all performance measures and all short and intermediate outcomes listed under DP23-0004, without modification. We understand our work will contribute to long-term outcomes of the NCHP, but we are not required to report on these long-term outcomes under this funding opportunity.

SD DOH will build upon the collaborative work achieved through prior Centers for Disease Control and Prevention (CDC) funding opportunities (i.e., 1305, 1815). Our LC, the SD Cardiovascular Collaborative, will facilitate communication and exchange of ideas between health systems, tribal organizations, community health organizations, quality improvement organizations, academic institutions, and public health entities.

Our LC boasts a demonstrated history of achieving sustainable change and improvement in the areas prioritized under DP23-0004. The strong partnership with the South Dakota Cardiovascular Collaborative and related partners, American Heart Association (AHA), South Dakota State University College of Pharmacy, Community Pharmacy Enhanced Services Network (CPESN), Office of Maternal and Child Health (MCH), Community Health Worker Collaborative of South Dakota (CHWSD), and the South Dakota Community Information Exchange (SD CIE) will provide a broad statewide base to reach high burden and priority populations through utilization of evidence-based practices and training.
