Grant Description
Wyoming's Cardiovascular Health Program - the Wyoming Department of Health-Public Health Division (WDH-PHD) whose address is: 122 West 25th Street, 3rd Floor West, Cheyenne, Wyoming 82002 - will be applying for CDC-RFA-DP-23-0004, the National Cardiovascular Health Program.
WDH-PHD is committed to promoting, protecting, and improving health of the population and communities in Wyoming. WDH-PHD will use this funding to develop and maintain partnerships with key stakeholders across the state in order to implement and evaluate evidence-based and evidenced informed strategies that decrease risk for cardiovascular disease, with a focus on hypertension and high cholesterol, among adults at high risk.
Project activities will be completed under the required strategies, which focus on tracking and monitoring clinical and social services and support needs measures shown to improve health and wellness, health care quality, and identify patients at highest risk of cardiovascular disease (CVD); implement team-based care to prevent and reduce CVD risk with a focus on hypertension and high cholesterol prevention, detection, control, and management through the mitigation of social support barriers to improve outcomes; and link community resources and clinical services that support bidirectional referrals, self-management, and lifestyle change to address social determinants that put the priority populations at increased risk for CVD.
Specific outcomes to be achieved by this project in year one include:
1) Increased use of electronic health records (EHRs) or health information technology (HIT) to report, monitor, and track clinical and social services and support needs data;
2) Increased use of standardized processes or tools to identify, assess, track, and address the social services and support needs;
3) Increased use of EHRs or HIT to support communication and coordination among care team members;
4) Increased use of multidisciplinary care teams adhering to evidence-based guidelines;
5) Increased multidisciplinary partnerships that address identified barriers to social services and support needs;
6) Increased community-clinical links to identify and respond to social services and support needs;
7) Increased engagement of community health workers to provide a continuum of care; and
8) Increased use of self-measured blood pressure monitoring with clinical support.
