Eight percent of Oregon adults have been diagnosed with diabetes, and 1.1 million are estimated to have prediabetes. In 2021, diabetes was the eighth-leading cause of death in Oregon, and annual direct and indirect medical expenses for people with diabetes is in the billions of dollars.

Approximately 25% of Oregonians (1.4 million people) are Medicaid recipients who, in general, suffer higher rates of diabetes (13%) compared to non-Medicaid members (8%). Likewise, the prevalence of diabetes among Native American and Alaskan Native (AI/AN) and Latino/a populations is 13% and 14% respectively, compared to 8% for White, non-Latino/a Oregonians.

The disparities that exist around diabetes and other chronic diseases are rooted in the social determinants of health (SDOH), which are the conditions that people are born into and live in.

The Health Promotion and Chronic Disease Prevention (HPCDP) section of the Oregon Health Authority’s Public Health Division is located at 800 NE Oregon St, Portland, OR 97232. Through the systems, infrastructure, partnerships, and authority granted by the state of Oregon, HPCDP has the capability to serve all populations and communities in Oregon.

HPCDP is applying for Component A of the CDC’s grant CDC-RFA-DP-23-0020. Through this grant opportunity, HPCDP will use evidence-based strategies to decrease risk for type 2 diabetes and improve diabetes self-care practices, while supporting an increase in capability of the state’s diabetes workforce to address SDOH's.

The work will include a focus on bolstering Oregon’s community health worker infrastructure to provide greater access to culturally appropriate services in all communities. HPCDP will also increase access, feasibility, and enrollment in diabetes self-management education services and the National Diabetes Prevention Program, strengthen state infrastructure and services around e-referrals and community-clinical linkages, umbrella hub arrangements, and other third-party billing infrastructures, which will enable community-based organizations and community-based healthcare providers to support the health of Oregon communities in a sustainable and equitable way.

Work will be focused primarily on Medicaid recipients and members of the AI/AN and Latino/a communities.
