Grant Description
A Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes - The California Department of Public Health (CDPH) is a state health department applying for the Centers for Disease Control and Prevention funding opportunity, CDC-RFA-DP23-0020, National Center for Chronic Disease Prevention and Health Promotion, a Strategic Approach to Advancing Health Equity for Priority Populations with or at Risk for Diabetes, Component A.

The proposed area of focus is the state of California, where CDPH has the tools, commitment, and resources needed to serve all Californian populations and communities in this grant. CDPH will implement activities for Component A that will decrease the risk for type 2 diabetes among priority populations, enhance quality of care and early detection of diabetes complications, and support family-centered childhood obesity interventions.

To ensure CDPH implements health equity solutions throughout all phases of this grant, CDPH will work closely with other state agencies, local health departments, and other key community stakeholders and partners. CDPH will be thorough in its approach to seek and implement feedback from within priority populations.

CDPH will implement the selected strategies in areas with disproportionately lower incomes and minority communities with high risk factors for prediabetes and diabetes prevalence, such as inaccessibility to physical activity, healthy foods, and equitable employment and education. Strategy implementation in these areas will support public health efforts in the prevention and management of diabetes across all community settings and health system organizations.

CDPH will focus on improving social determinants of health that will result in better outcomes for health equity for priority populations. CDPH will use funding from this grant to address prevention and management of diabetes and its associated risk factors. Staff will promote awareness and reporting of diabetes and prediabetes among at-risk populations to improve health management and health outcomes.

CDPH will collaborate with health care plans and systems, leverage ongoing statewide initiatives for community health worker expansion and reimbursement, and integrate pharmacist networks to improve team-based care while encouraging healthy lifestyle changes. CDPH will also monitor and evaluate the effectiveness of the proposed multi-level, coordinated approach with health equity integration to achieve and deliver sustainable impact on diabetes prevention and management, and improve the long-term health of all Californians.
