Grant Description
Louisiana Actions to Achieve Health Equity to Control Diabetes - Objective: Well-Ahead Louisiana's (WAL) Louisiana Actions to Advance Health Equity to Control Diabetes project proposal will work to decrease risk for type 2 diabetes among adults with prediabetes and improve self-care practices, quality of care, and early detection of complications among people with diabetes. It will also implement evidence-based, family-centered childhood obesity interventions as a type 2 diabetes risk reduction strategy.

Project Proposal: WAL proposes to use statewide and regional strategies to advance health equity for priority populations with or at risk for diabetes in the state of Louisiana. The project will address the social determinants of health (SDOH) that contribute to the disproportionate burden of diabetes in these populations and implement strategies to improve access to care and resources. WAL has demonstrated its ability to reach and serve these populations through previous funding opportunities and will continue to build on that foundation through continued partnerships with community-based organizations, healthcare facilities, and other organizations located in these communities. WAL has chosen priority populations for this work not only based on need, but also considering the strength of its existing partnerships in the region.

Purpose: The purpose of WAL's proposal is to improve the health of Louisiana residents who have disproportionately been burdened with diabetes and pre-diabetes by improving the landscape of diabetes management and prevention.

Outcomes: WAL expects to achieve the following outcomes related to diabetes management and type 2 diabetes prevention as a result of this program:

Short Term:
1. Increased number of organizations implementing evidence-based community behavioral change programs;
2. Increased adaptation/tailoring of effective programs for priority populations;
3. Increased number of patients screened and referred to community resources;
4. Increased SDOH screenings in clinical settings.

Intermediate:
1. Increased participation in evidence-based community behavioral change programs;
2. Increased multi-directional communication between clinical and community resources;
3. Improved self-management of chronic diseases;
4. Increased proportion of the population receiving the health care services they need and want.

Long Term:
1. Decreased proportion of people with diabetes with an A1C > 9%;
2. Increased number of program completers served by CDC-recognized National DPP delivery organizations who reduce their risk for type 2 diabetes;
3. Decreased percent of the 95th percentile body mass index (BMI) and percent of median BMI in children, improvement in pediatric quality of life, and decrease in BMI among caregivers.

Evaluation: WAL will allocate 13% of total funding to its internal epidemiology and evaluation team to carry out the evaluation activities associated with this funding. Evaluation and performance measure planning will be done in a collaborative manner with program staff and partners. Internally, evaluation findings will inform WAL's overall strategy and practices around diabetes prevention and management. Externally, WAL will disseminate evaluation findings to stakeholders through informal check-in meetings, presentations and/or briefs, through the WAL public website, www.wellaheadla.com, in addition to the evaluation reporting requirements to the CDC.

Key Personnel: Key personnel from WAL include Division Manager for Tobacco Control and Disease Prevention, Diabetes Program Manager, Chronic Disease Epidemiologist, Chronic Disease Evaluator, Healthcare Innovation Manager, Diabetes Coordinator, Communications Assistant, Budget Manager, and Administrative Program Specialist.
