Grant Description
A strategic approach to advancing health equity for priority populations with or at risk for diabetes - The Delaware Division of Public Health’s Diabetes and Heart Disease Prevention and Control Program aims to develop and implement a strategic approach to promoting health equity for priority populations with or at risk for diabetes. The focus will be on identifying and addressing disparities in access to care, quality of care, and outcomes related to prediabetes and diabetes.

The project will involve collaboration with relevant stakeholders, including healthcare providers, community-based organizations, to identify and prioritize actions that will have the greatest impact on improving health equity for these populations. The goal is to improve the health and well-being of individuals with or at risk for diabetes and to reduce disparities in diabetes outcomes.

Delaware will continue to build capacity and sustainability around Category A work for diabetes management and type 2 diabetes prevention implemented during the 5-year funding cycle with 1815. During the first year of the DP-23-0020 grant, DPH will work diligently to address strategies and measures defined in the application.

- Increase in the number of organizations implementing evidence-based community behavioral change programs.
- Increased adaptation/tailoring of effective programs for priority populations.
- Increase in the number of patients screened and referred to community resources.
- Increased SDOH screenings in clinical settings.
- Increased participation in evidence-based community behavioral change programs.
- Increased multi-directional communication between clinical and community resources.

Delaware has been successful in several areas of healthcare, population health, and improving access to diabetes care in underserved areas. Building on its strong internal infrastructure and unique relationship with a diverse set of partners, the Delaware Division of Public Health is prepared to initiate the development and implementation of the proposed initiatives.
