Grant Description
COVID and Beyond - Active Community Engagement (ACE Project)

This grant project seeks to actively engage at-risk and minority community members in their own healthcare regarding COVID-19 and other health issues. Using Community Health Workers (CHWs) and community partners to help build resiliency, this grant seeks to strengthen a community's sense of efficacy surrounding how to best manage a variety of healthcare concerns and social determinants that impact health concerns, including COVID-19.

OCCHD, in partnership with local clinics and community organizations, plans to hire and train community health workers who will be active members of the communities in which they work. Implementing a competency-based training protocol for the CHWs as well as the community partner organizations and the community champions will help to build a sense of efficacy around the community's ability to address health concerns.

Key to the approach of this grant's efforts is community involvement and input to help ensure that communities' needs and desires are being addressed and that the interventions are culturally and linguistically appropriate, meeting the CLAS standards for health services for each of the populations being served.

By addressing COVID-19 concerns through a social determinants lens and also examining other concomitant health concerns, the programs put into place for COVID-19 will be sustainable beyond the pandemic period.
