Grant Description
National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities - The Illinois Department of Public Health (IDPH) has developed a grant proposal to successfully implement CDC-RFA-OT21-2103, the National Initiative to Address COVID-19 Health Disparities Among Populations at High-Risk and Underserved, Including Racial and Ethnic Minority Populations and Rural Communities.

The grant will fund efforts from various offices at the IDPH to deliver strategy-specific program and evaluation deliverables. Our programs include integration of stakeholders encompassing statewide efforts as well as rural-specific providers.

The focus of the proposed strategy outcomes will rely on reducing COVID-19 health disparities, improving and increasing contact tracing, and improving health department capacity and services to prevent and control COVID-19 infection.

It is expected that this grant application will strengthen collaboration between IDPH and community partners to reach targeted Illinois residents that were most impacted during this pandemic, as well as improve health outcomes due to integrated COVID equity efforts.
